| Monthly Maintenance Fees |
A maintenance fee of $6.00 will be imposed every statement cycle if the balance in the account falls below $1,500.00 any day of the monthly statement cycle.
A transaction fee of $0.15 will be imposed for each item deposited in excess of 150 per the monthly statement cycle regardless of balance.
A transaction fee of $0.15 will be imposed for each debit in excess of 50 transactions (withdrawal, check paid, automatic transfer or payment out of your account) per the monthly statement cycle regardless of balance. |

- Updated proof of address if not correct on driver's license
